What could a PC triple fault when he?s jumoing to PMode? I have a right GDT and my jump is also OK! What else could it be?

A triple fault occurs when:
- some invalid info in GDT,
- bad IDT setup
- you try to access an invalid selector (for example load a data selector into CS),
- push/pop without a valid stack pointer,
- invalid opcode,
- DOUBLE fault exception not handled,
- ... i don't remember other things..

Ok, maybe someone have a look at the code. It is a bootstrap for my own filesystem. So if you don?t understand why I?m doing something in my bootstrap, please ask!
Edit::
Maybe my problem is not clear! The bootstrap is triple faulting.
you can download it at :

Ok I uploaded my old bootstrap which is working! Although there aren?t many changes (hard changes) the new code is triple faulting.
