Why are there two "Forum" words in the link to the index page?

I assume theres just a missing separator:

OSDev Forum -> Forum Index

or that when Chase set the forum up, he entered 'OSDev Forum' in the forum name field, instead of just 'OSDev'.

AJ is correct.

"OSDev Forum" was entered as the forum name. I've made the same mistake when I set up a forum.
